Revision of the Forms and Documents Related to the Regulations Governing Subsidies from Special Municipalities and County (City) Governments for Assisting Workers Suffering from Occupational Accidents to Return to Work

This applies to special municipality and county (city) governments administering subsidies for assisting workers suffering occupational accidents to return to work, as well as employers and such workers applying for subsidies. It revises the application, roster, personal-data authorization, reimbursement, and follow-up record forms for measures including assistive-facility subsidies, vocational rehabilitation allowances, and subsidies for employing workers suffering occupational accidents. Employers applying for assistive-facility subsidies must submit worker rosters, proof of employment, return-to-work plans or assessment reports, before-and-after improvement photographs, and expenditure receipts. Local governments may conduct document reviews, review meetings, or on-site visits and must continue to follow up on facility use, employment, and workers’ return-to-work status. The forms also regulate authorization for the collection, processing, use, and verification of personal data.
